About the Role

This is an excellent opportunity to lead on the management and ongoing development of a highly successful integrated multi-disciplinary service, offering Intermediate care and Reablement to residents of the community.

The Integrated Independence Team is well established and offers a range of interventions through its multiple integrated therapy-led pathways of Rapid Response and Home Treatment, as well as a mobile morning falls service and the care delivery services of Reablement (which is delivered on behalf of the local authority) and Rapid Care.

Main Responsibilities
  • Support the work of the whole service and provide supervision and guidance to the team leads and senior social worker.
  • Seek new opportunities for development and improvement of the service.
  • Lead on the proactive engagement with commissioners and ensure the relationship with the local authority remains positive and productive.
  • Ensure performance and quality reporting is undertaken with accuracy and to manage the complex funding and budgetary arrangement effectively.
  • Advise on practice and complex clinical, social or practice issues in line with own professional registration.

Person Specification

The ideal candidate will have:

  • Current UK Professional Registration.
  • Evidence of ongoing professional development.
  • Substantial experience of managing a multidisciplinary health and / or social care team.
  • Excellent written communication skills.
  • Demonstrating an understanding of Trust Values.
  • Masters degree or equivalent level of expertise and experience.
  • Successfully implement change.
  • Experience of working in and delivering community based services.
  • Experience of engaging health and/or social care commissioners.
