About the Role

We are seeking a dedicated and compassionate Care Support Specialist to join our team at Independent Community Care Management Limited.

As a Care Support Specialist, you will work closely with our clients and their families to understand their unique needs and preferences, and provide personalized care and support to enhance their quality of life.

You will have the opportunity to work in a variety of settings, including clients' homes and community-based locations, and will be supported by our experienced team of healthcare professionals.

Key Responsibilities:

  • Provide personalized care and support to clients with complex needs
  • Work closely with clients and their families to understand their needs and preferences
  • Develop and implement care plans that meet the unique needs of each client
  • Provide emotional support and companionship to clients
  • Collaborate with other healthcare professionals to ensure seamless care and support

Requirements:

  • Previous experience in care and support work is not necessary, as we provide full training and clinical support
  • Excellent communication and interpersonal skills
  • Ability to work independently and as part of a team
