We are seeking a highly motivated and compassionate Community Support Worker to join our team in the Personality Disorder Pathway Hub. As a key member of our multidisciplinary team, you will provide high-quality care to individuals with complex emotional needs, working closely with nurses, medics, and other healthcare professionals to build positive relationships and engage clients in their care plans.
The successful candidate will have a basic understanding of mental health conditions, a caring approach to patients, and the ability to work as part of a team. You will be responsible for implementing patient care plans, co-facilitating therapy groups, and providing support with consolidation of skills learned during therapy. A full UK driving licence and vehicle for business use are required for this post, with reasonable adjustments made for disabled individuals in line with the Equality Act 2010.
Key Responsibilities- Provide support to patients with complex emotional needs, working closely with the multidisciplinary team to build positive relationships and engage clients in their care plans.
- Implement patient care plans, co-facilitating therapy groups and providing support with consolidation of skills learned during therapy.
- Work closely with nurses, medics, and other healthcare professionals to provide high-quality care to patients.
- Develop observation skills with the ability to report and record physical/mental health changes, incidents, events, and behaviours in an accurate and timely manner.
- NVQ level 3 Health and Social Care Certificate or equivalent experience.
- Basic understanding of mental health conditions.
- Caring approach to patients.
- Ability to work as part of a team.
- Diplomatic, caring, and sympathetic.
