About the Role
We are seeking a dedicated and skilled Nurse Associate to join our Primary Care Network. As a key member of our multidisciplinary team, you will play a vital role in delivering high-quality care to our patients.
Main Responsibilities
- Work collaboratively with the clinical team to provide comprehensive care across all ages.
- Assist with the assessment, planning, implementation, and evaluation of nursing care plans.
- Conduct routine health checks, administer medications, and assist with minor procedures.
- Perform diagnostic tests and assist in the collection and interpretation of data, including vital signs and patient histories.
- Provide health education and advice to patients and their families, promoting healthy lifestyles.
- Maintain accurate and up-to-date patient records, ensuring confidentiality.
- Coordinate and facilitate referrals to specialist services, ensuring timely access to care.
- Participate in quality improvement initiatives to enhance patient care outcomes.
- Keep up-to-date with developments in healthcare and attend relevant training sessions.
