Job Title: Palliative Care Specialist Nurse

We are seeking a highly skilled and compassionate Palliative Care Specialist Nurse to join our team at The Prince & Princess of Wales Hospice. As a Palliative Care Specialist Nurse, you will be responsible for providing specialist palliative care support and advice to patients and their families in their own homes.

Key Responsibilities:

  • Provide specialist palliative care support and advice to patients and their families in their own homes.
  • Work collaboratively with the multi-disciplinary team to provide comprehensive care to patients.
  • Develop and implement individualized care plans to meet the unique needs of each patient.
  • Provide education and support to patients and their families on palliative care and end-of-life care.
  • Participate in quality improvement initiatives to enhance the quality of care provided.

Requirements:

  • Registered Nurse with a minimum of 2 years' experience in palliative care.
  • Current, clean driving license and own transport.
  • Ability to work autonomously and as part of a team.
  • Excellent communication and interpersonal skills.
  • Ability to provide person-centered care and support to patients and their families.
