About the Role

We are seeking a highly skilled and experienced Senior Community Nurse to join our team at NELFT North East London Foundation Trust. As a Senior Community Nurse, you will play a key role in leading and managing a Community Nursing Team, providing high-quality care to patients and their families in the community.

Key Responsibilities
  • Lead and manage a Community Nursing Team, ensuring the provision of a high standard of nursing care to patients and their families in the community.
  • Work as part of the integrated primary/community health care team, collaborating with GPs, other professionals, and statutory and voluntary agencies to deliver individualized care to patients.
  • Provide leadership to the community nursing team, working without direct supervision and ensuring the effective management of a caseload.
  • Support and supervise pre-registration students, providing guidance and mentorship to help them develop their skills and knowledge.
  • Collaborate with other health and social professionals to ensure seamless care delivery and effective communication.
Requirements
  • Current NMC Registration as a Registered Nurse (RN)
  • Evidence of professional up-dating and a commitment to ongoing learning and development
  • Agreement to undertake the Band 6 Development Program or equivalent
  • Academic ability to study at degree level
  • Relevant post-registration course and experience in Palliative Care, Diabetes, and Tissue Viability (desirable)
  • Experience of team working, interagency and collaborative working, teaching and assessing individuals, and providing complex packages of care (essential)
  • 2 years of community experience and a good range of clinical skills, including assessment (essential)
