About the Role

We are seeking a dedicated and compassionate Support Practitioner to join our team at Cornerstone. As a Support Practitioner, you will play a vital role in providing high-quality care and support to individuals with varying needs.

Key Responsibilities
  • Support individuals to manage their daily lives, including personal care, medication, and household tasks.
  • Encourage and support individuals to engage in meaningful activities, hobbies, and interests.
  • Develop and maintain positive relationships with individuals, their families, and other stakeholders.
  • Work collaboratively with other professionals to ensure comprehensive care and support.
  • Maintain accurate and up-to-date records of individual's care and support plans.
Requirements
  • Excellent communication and interpersonal skills.
  • Ability to work independently and as part of a team.
  • Compassion, empathy, and understanding of the needs of individuals with varying abilities.
  • Ability to maintain confidentiality and adhere to data protection policies.
  • Flexibility to work varied shifts, including evenings, weekends, and sleepovers.
