Key Responsibilities- Develop and implement personalized lifestyle support plans to promote slow stream rehabilitation and relapse prevention.
- Work collaboratively with residents, families, and healthcare professionals to ensure seamless care and support.
- Lead the delivery of wellbeing activities and programs, focusing on stimulating the body, mind, and soul.
- Collaborate with the Named Practitioner Triangulation Team to ensure effective case review and planning.
- Conduct regular assessments and evaluations to inform and adjust wellbeing plans.
- Background in working with vulnerable individuals, preferably with experience in alcohol-related brain damage.
- Knowledge and experience in developing wellbeing programs and promoting meaningful occupation and social engagement.
- Excellent communication and interpersonal skills, with the ability to build strong relationships with residents, families, and colleagues.
- Ability to work independently and as part of a team, with a focus on delivering high-quality care and support.
