As a specialist in rehabilitation and relapse prevention, you will play a vital role in supporting individuals with alcohol-related brain damage to achieve their full potential and live a meaningful life.

Key Responsibilities
  • Develop and implement personalized lifestyle support plans to promote slow stream rehabilitation and relapse prevention.
  • Work collaboratively with residents, families, and healthcare professionals to ensure seamless care and support.
  • Lead the delivery of wellbeing activities and programs, focusing on stimulating the body, mind, and soul.
  • Collaborate with the Named Practitioner Triangulation Team to ensure effective case review and planning.
  • Conduct regular assessments and evaluations to inform and adjust wellbeing plans.
Requirements
  • Background in working with vulnerable individuals, preferably with experience in alcohol-related brain damage.
  • Knowledge and experience in developing wellbeing programs and promoting meaningful occupation and social engagement.
  • Excellent communication and interpersonal skills, with the ability to build strong relationships with residents, families, and colleagues.
  • Ability to work independently and as part of a team, with a focus on delivering high-quality care and support.
