About the Role

We are seeking a highly motivated and compassionate Care and Support Officer to join our Specialist Support for Younger Adults team at North Northamptonshire Council.

As a Care and Support Officer, you will work in a community-based setting, supporting individuals with various disabilities and needs. Your primary responsibility will be to conduct strength-based assessments, identify care needs, and deliver support plans in collaboration with individuals, carers, and multi-agencies.

You will be required to assess and manage risks effectively, provide accurate information and support, and manage your caseload to ensure timely assessments and reviews. A person-centred approach, excellent communication skills, and the ability to work in partnership with other agencies are essential for this role.

Key Responsibilities
  • Conduct strength-based assessments and identify care needs

Requirements
  • GCSE or equivalent qualification
  • Care Certificate or willingness to work towards
  • Level 2 or above in Health and Social Care or equivalent
  • Proven experience in health or care work
  • Experience of delivering care services in a community-based setting
