Home Instead is seeking a dedicated and experienced Care Management Support Specialist to join our team. This role is essential in supporting our care management operations, ensuring our services are delivered efficiently and to the highest standards.
Key Responsibilities:
- Care Consultations: Conduct initial consultations with potential clients and their families to assess their needs and preferences. Support with the development of comprehensive care plans tailored to individual client requirements.
- Care Planning: Collaborate with the care team to create, implement, and update personalized care plans. Ensure all care plans are regularly reviewed and adjusted as needed to meet clients' evolving needs.
- Quality Assurance: Monitor and evaluate the quality of care provided, ensuring it meets Home Instead's high standards. Conduct regular service reviews and implement improvements based on feedback and assessments.
- Service Reviews: Support routine reviews of care services to ensure client satisfaction and compliance with care plans. Address any concerns or issues promptly and effectively.
- Safe Scheduling: Support the scheduling of care visits to ensure optimal coverage and continuity of care. Coordinate with caregivers to handle any scheduling changes or emergencies.
- Compliance: Ensure all care activities comply with relevant regulations, policies, and Home Instead standards. Maintain accurate and up-to-date documentation and records.
- Field Support: Provide on-the-ground support to Care Professionals and clients, addressing any immediate issues or needs. Conduct home visits to monitor the quality of care and support provided.
- On Call: On rotation with others within the team.
Requirements:
- Proven experience in domiciliary care, preferably within a franchise environment.
- Strong understanding of care planning, quality assurance, and compliance requirements.
- Excellent communication and interpersonal skills.
- Willingness to carry out on-call duties on a rotational basis.
- Ability to manage multiple tasks and priorities effectively.
- Strong organizational and problem-solving skills.
- Valid driver's license and own car (essential for field support duties).
- Right to Work in the UK must be established.
