Social Worker for Children with Disabilities Team

Job Summary: We are seeking a skilled Social Worker to join our Children with Disabilities Team. As a key member of our team, you will work closely with children and young people who have complex needs, including physical and developmental disabilities.
Key Responsibilities:
- Assess the needs of children and young people with disabilities and develop individualised care plans.
- Facilitate access to services that enhance independence, well-being, and quality of life.
- Work collaboratively with families, carers, and other professionals to ensure comprehensive support.
- Monitor and evaluate the effectiveness of care plans and make adjustments as needed.
Requirements:
- Qualification in Social Work or a related field.
- Experience working with children and young people with disabilities.
- Strong communication and interpersonal skills.
- Ability to work independently and as part of a team.
