About the Role

We are seeking a motivated and compassionate Therapy Assistant Practitioner to join our team at Leeds Community Healthcare NHS Trust. As a key member of our multidisciplinary team, you will play a vital role in providing high-quality care to patients in our rehab and recovery hubs across Leeds.

Key Responsibilities

  • Work collaboratively with registered therapists to deliver evidence-based therapeutic interventions
  • Support patients in achieving their maximum dependency state, using therapy outcome measures to demonstrate success
  • Participate in daily MDTs to discuss patient progress and develop individualized care plans
  • Ensure compliance with policies and procedures, and maintain a high standard of clinical work
