We are seeking a skilled and dedicated Community Health Nurse to join our team in the Evening Shift. As a Community Health Nurse, you will be working as an autonomous practitioner within our Outpatient Services team.
Main Responsibilities- Provide high-quality nursing care and interventions to patients in their own homes, with support from team leaders.
- Assess, plan, and implement nursing care to patients, ensuring patient and carer participation in decision-making.
- Accountable for assessing, interpreting, planning, implementing, and evaluating treatment to patients within professional guidelines.
- Identify patient needs through holistic nursing and develop care plans accordingly.
- RGN level 1 and RGN level 2 qualifications.
- Evidence of professional development.
- Experience in an acute or primary care setting, although newly qualified staff may be considered with relevant placement experience.
- Ability to demonstrate commitment to high-quality care and service provision.
- Ability to assess, plan, and implement nursing care.
- Ability to safeguard and promote the welfare of children and young people/vulnerable adults.
- Ability to travel across the county sometimes at short notice.
