Job Title: Clinical Lead Nurse Specialist

Job Summary:

Leaders in Care is seeking an experienced Paediatric Nurse with a passion for delivering complex care to join our team as a Clinical Lead Nurse Specialist. As a key member of our care team, you will be responsible for the overall provision of complex care, creating personalised care plans for each service user, and working closely with a core team of carers to train and support them with their clinical skills and activities.

Key Responsibilities:

  • Design and deliver tailored care plans specific to an individual client's needs
  • Provide ongoing monitoring, support, and assessment of clients' physical and physiological needs within your caseload
  • Work closely with community teams, CHC nurses, and CCG commissioners to establish and maintain bespoke packages of care
  • Collaborate with the care team to ensure high-quality care delivery and continuous improvement

Requirements:

  • Be an RSCN with a minimum of 2 years' experience in the UK with an active NMC PIN
  • Have up-to-date experience of tracheostomy care and ventilators
  • A full UK Driving Licence is essential
