Job Title: Service Lead for Neuro Community Services

We are seeking an experienced and skilled Service Lead to join our team at The Walton Centre NHS Foundation Trust. As a Service Lead, you will be responsible for leading and managing a clinical and administration team to effectively perform key functions of rehabilitation coordination, single point of contact, and bed management.

About the Role

The successful candidate will have a strong background in rehabilitation, with experience in leading a team and managing staff. You will be responsible for ensuring the Rehabilitation Coordination Service operates in a safe, effective, and efficient way as part of the whole network. This will involve facilitating proactive links and collaborative relationships between the Service and the wider system to support clinically appropriate and timely referrals.

You will also be responsible for ensuring effective communications systems are established, maintained, and monitored regularly within the Service, between teams, and across referring services/organizations. Additionally, you will be responsible for ensuring appropriate and timely transfer of care across network services and maintaining an accurate database.

Key Responsibilities
  1. Lead and manage a clinical and administration team to effectively perform key functions of rehabilitation coordination, single point of contact, and bed management.
  2. Ensure the Rehabilitation Coordination Service operates in a safe, effective, and efficient way as part of the whole network.
  3. Facilitate proactive links and collaborative relationships between the Service and the wider system to support clinically appropriate and timely referrals.
  4. Ensure effective communications systems are established, maintained, and monitored regularly within the Service, between teams, and across referring services/organizations.
  5. Ensure appropriate and timely transfer of care across network services.
  6. Maintain an accurate database.
  7. Provide line management, including management of performance, for staff within the Service.
  8. Ensure arrangements are in place for all staff members to receive regular supervision, appraisal, and have their personal development plans and training needs identified and where possible met.
  9. Responsible for service performance, including monitoring of the delivery of targets and reporting as appropriate, meeting internal and external information requirements, and data quality.
  10. Lead and motivate staff to provide a proactive and responsive high-quality service, contributing to ongoing audit, service improvement, and evaluation.
  11. Represent the Service at key meetings.
Requirements

The successful candidate will have:

Nursing or AHP degree or equivalent relevant professional qualification.Educated to Master's degree or proven equivalent knowledge within relevant specialism.Leadership/Management qualification or proven equivalent knowledge.Demonstrable postgraduate experience in a rehabilitation setting.Previous experience as a nurse or allied health professional with extensive experience of treating patients with neurological and/or multiple conditions.Clinically relevant experience in neurology or trauma specialties within a relevant rehabilitation setting.Experience of leading a team and managing staff.Experience in performance management.Evidence of delivering on key quality performance indicators.Ability to demonstrate effective team working within a multi-agency/network framework.Demonstrable experience in evidence-based and reflective practice.To be able to communicate in an effective and persuasive manner at all times, even under pressure.To have highly developed interpersonal skills.Must be able to work as an autonomous practitioner.Understanding of patient flow.Understanding of national and local policies and guidance.Demonstrates an understanding of the rehabilitation ethos and has a high level of ability that will inform and enhance optimum recovery and quality of life.Understanding of the Mental Health Act 1983 and Mental Capacity Act.Managing and interpreting data.Participation in change management.Ability to use the Microsoft Office software package.Understanding of clinical audit and research practices relevant to rehabilitation.Demonstrable knowledge and experience of a range of rehabilitation functional outcomes measures and implications for long-term recovery of life skills.
