About the Role
We are seeking a skilled and dedicated Community Health Nurse to join our team at Dorset Clinical Commissioning Group. As a Community Health Nurse, you will play a vital role in providing high-quality patient care and support to individuals in their own homes.
Key Responsibilities
- Provide comprehensive nursing care to patients in their own homes, including assessments, treatments, and education.
- Work collaboratively with other healthcare professionals to ensure seamless care coordination and delivery.
- Develop and maintain strong relationships with patients, families, and carers to promote patient-centered care.
- Contribute to the development and implementation of care plans that meet the unique needs of each patient.
- Participate in quality improvement initiatives to enhance patient outcomes and experiences.
Requirements
- Registered practitioner to degree/diploma level.
- Membership of the relevant Professional Body.
- Learning and Assessing in Practice Qualification or equivalent practice assessors training.
- Evidence of recent professional development in an up-to-date portfolio.
- Able to communicate effectively at different levels of the organisation and with staff, patient/service users, visitors or external organisations both verbally and in writing in the exchange of complex, sensitive or contentious information which may require the use of negotiating and/or persuasive skills.
- Ability to use own initiative within sphere of authority.
- Willingness to advance own clinical knowledge, skill and competence based on current evidence.
- Able to analyse and assess situations and to interpret potentially conflicting situations and determine appropriate action, where there is a range of options and judgement is required.
- Supervised experience gained during clinical placements.
- Ability to mentor students / supervise other junior staff.
- Trained in PMVA techniques or willing to be trained.
- Demonstrable knowledge of assessment and therapeutic interventions in area of practice.
Desirable Criteria
- Recent previous experience within a comparable role.
- Advanced wound care experience, end of life care.
