The West Yorkshire Keyworker Service is expanding its offerings, and the establishment of the West Yorkshire Neurodiversity Family Hub aims to provide early assistance to neurodiverse individuals up to the age of 25 and their families within their communities.

Our mission is to support families throughout their journey, whether they are seeking help before, during, or after a neurodevelopmental assessment. We aim to empower families to navigate the system, promote collaborative decision-making, and ensure they are informed about available resources. Support will be tailored to individual needs and strengths identified post-assessment.

Our goal is to ensure that individuals and families feel equipped to make informed decisions and know where to find early support to enhance their emotional wellbeing and prevent crises.

We will:

Align with existing Neurodevelopmental Family Navigator services in Bradford, Calderdale, Kirklees, and Leeds, focusing on effective waiting strategies. Implement a Self-referral pathway to empower families and simplify the referral process for professionals. Assist parents/carers in referring children and young adults to the Dynamic Support Register based on identified needs. Provide a hybrid approach for both in-person and online family support. Facilitate group sessions to foster peer support opportunities. Offer strengths-based guidance, referrals, and crisis de-escalation. Collaborate with local Parent Carer Forums to enhance support. Develop peer support networks in conjunction with local and regional Parent Carer Forums. Where applicable, provide information on Right to Choose.

As a Neurodevelopmental Coordinator, your responsibilities will include:

Being aspirational for every child, young person, and their families. Demonstrating a passion for enabling individuals to achieve their desired potential. Being approachable and non-judgmental. Practicing respect and person-centered care. Exhibiting compassion and creativity in your approach. Maintaining honesty and transparency. Acting as an effective facilitator. Treating parents/carers as partners in the process. Providing peer support to parents and caregivers. Possessing knowledge of local services and facilitating access to support. Valuing lived experiences and actively listening to parents in need.

Key Functions of the Neurodevelopmental Coordinator:

Coordinating services at the local level and across West Yorkshire:

Identifying key areas of need and developing a schedule of support sessions for parents and caregivers, delivered by the team and multi-sector partners. Ensuring health, social care, education, and voluntary sector services are linked to neurodiversity. Leading a multi-sector delivery group to enhance service development and learning.

A valid driver's license and access to a vehicle for work purposes are essential for this role.
