Job Description

Job Title: Primary Care Diabetes

Job Summary:

We are seeking a highly skilled and dedicated individual to join our team as a Primary Care Diabetes Specialist. The successful candidate will be responsible for working with patients to ensure they receive the same level of care and that vulnerable patients are managed and followed up.

Key Responsibilities:

  • Use available population data to identify patients who require additional support and develop a plan to ensure they receive the same level of care as others.
  • Work with patients to understand their needs and provide guidance on how to access local services and resources.
  • Support patients to receive holistic care and signpost them to suitable care providers within the Primary Care Network (PCN).
  • Develop an understanding of local services available for patients with long-term conditions and signpost patients to these services.
  • Support carers of patients with long-term conditions and provide guidance on how to access support services.
  • Participate in Multi-Disciplinary Teams (MDTs) within the PCN to ensure effective communication and collaboration with other healthcare professionals.

Requirements:

  • Strong understanding of population health management and patient care.
  • Excellent communication and interpersonal skills.
  • Ability to work effectively in a team environment.
  • Strong analytical and problem-solving skills.
  • Ability to maintain confidentiality and handle sensitive information.

Working Arrangements:

This is a full-time position, working within the Primary Care Network (PCN). The successful candidate will be required to work flexibly to meet the needs of the service.
