Job Description:
We are seeking a skilled and dedicated Social Worker to join our team in Newcastle. As a Social Worker, you will be responsible for conducting comprehensive assessments, planning interventions, and working collaboratively to support and empower children in care and care leavers.
Responsibilities:
- Conduct thorough assessments of children in care and care leavers to identify risks, needs, and strengths.
- Develop and implement care plans to support the safety, well-being, and successful transition to independence of children in care and care leavers.
- Collaborate with multi-disciplinary teams, external agencies, and community organizations to coordinate and improve care.
- Provide support and advocacy for children in care and care leavers, ensuring their voices are heard and their rights are protected.
- Maintain accurate and up-to-date records of assessments, interventions, and outcomes in line with legal and professional standards.
Requirements:
- Qualified Social Worker registered with Social Work England.
- Proven experience in a social work role, preferably within children and family services.
- Excellent communication and interpersonal skills, with the ability to build effective working relationships with children, families, and professionals.
- Knowledge of relevant legislation, policies, and procedures related to social care and safeguarding.
