About the Role:
Hunter AHP Resourcing is working alongside our professional Community Psychiatric Nurses who are looking for a CPN to join their Adult team. We are seeking a great and hard working CPN to join our team, if this is a job for you, we would like to hear from you.
Job Details:
- Job Title: CPN – Care Coordinator
- Band: 6
- Location: Greater Manchester
- Duration: 3 Months which can be ongoing
- Schedule: Full Time 37.5 hours
- Rate of pay: £25 to £27 per hour
Your Responsibilities:
- To be responsible for the co-ordination, commissioning and personal delivery of individual packages of care for a caseload of service users within CMHTs.
- To undertake initial assessments of new referrals to the service.
- To support the duty system.
Requirements and Qualifications:
- Current and valid registration with the NMC.
- Proven experience within a CPN recovery role.
- Strong communication and interpersonal skills.
- Ability to work independently and as part of a multidisciplinary team.
- Commitment to providing high-quality patient care.
- Previous experience in working with an Adults team.
