About the Role

We are seeking a highly motivated and experienced Community Staff Nurse to join our team at Birmingham Community Healthcare NHS FT. As a Community Staff Nurse, you will be responsible for delivering high-quality care to patients in their own homes, GP premises, and other community settings.

Key Responsibilities
  • Work as part of an integrated multidisciplinary team to provide care to patients and promote health and well-being.
  • Assess and evaluate health needs, particularly in areas such as palliative care, wound management, diabetes, and other long-term conditions.
  • Develop and implement care plans to achieve the best outcomes for patients.
  • Work closely with other members of the IMT, including GPs, Rapid Response, and Advanced Nurse Practitioners, to prevent inappropriate hospital admissions and facilitate hospital discharges.
Requirements
  • Registered Nurse Level 1 (Adult)
  • Mentor and assessor qualification or equivalent
  • Experience of working in a multi-disciplinary team setting
  • Ability to demonstrate sound knowledge and application of up to date evidence-based clinical practice
  • Excellent communication, analytical, and problem-solving skills
  • IT literate and aware of the complex needs of a multicultural, socially diverse population
  • Effective time management and ability to work under pressure
Working Arrangements

The service operates between 8 am and 6 pm, 7 days a week. The successful candidate will be required to work weekends and bank holidays. A full driving license and car are essential for this role.
