About the Role
We are seeking an experienced Occupational Therapist to join our team at Heart of Kent Hospice. As a Palliative Care Specialist, you will work with patients, their families, and carers to develop and deliver a coordinated and comprehensive palliative care service.
Key Responsibilities
- Work with patients to develop their independence and autonomy with activities of daily living, involving the patient, carer, family, and MDT in the treatment plan.
- Provide education on symptom management or equipment use.
- Provide adaptation/equipment as appropriate to aid daily living, and provide instruction in their safe and proper use in the Hospice and at home where appropriate.
- Take a lead role in the planning for discharge from the Inpatient Unit, ensuring community follow-through for patients with complex needs.
- Undertake home assessments as required to support IPU discharge.
- Plan and deliver sessions as part of the Hospice Hub in Magnolia Place Outpatient Centre.
- Be a specialist resource to other members of the multi-professional team, providing up-to-date information on equipment to promote patient independence.
- Communicate and work closely with all members of the multi-professional team, including complementary therapists and volunteers in the inpatient unit and Hospice Hub in Magnolia Place Outpatient Centre.
Person Specification
- Professional Registration with HCPC.
- Able to demonstrate up to date evidence-based practice.
- Team Player.
- Good Communication Skills.
