About the Role

We are seeking a highly skilled and compassionate Occupational Therapist to join our team at Heart of Kent Hospice. As a Palliative Care Specialist, you will play a vital role in providing expert care and support to patients and their families.

Key Responsibilities
  • Work with patients to develop their independence and autonomy with activities of daily living, involving the patient, carer, family, and multidisciplinary team in the treatment plan.
  • Provide education on symptom management and equipment use to patients and their carers.
  • Take a lead role in planning for discharge from the Inpatient Unit, ensuring community follow-through for patients with complex needs.
  • Undertake home assessments as required to support IPU discharge.
  • Plan and deliver sessions as part of the Hospice Hub in Magnolia Place Outpatient Centre.
  • Provide specialist resources to other members of the multidisciplinary team, promoting patient independence.
  • Communicate and work closely with all members of the multidisciplinary team, including complementary therapists and volunteers.
Person Specification
  • Professional Registration with HCPC.
  • Able to demonstrate up to date evidence-based practice.
  • Team player.
  • Good communication skills.
