Job Summary

We are seeking a highly skilled and compassionate Mental Health Support Specialist to join our team. As a key member of our rehabilitation service, you will play a vital role in supporting individuals with mental health and complex needs to achieve their goals and live fulfilling lives.

Key Responsibilities
  • Client Support and Well-being: Provide person-centered support and guidance to clients, using key-working and support planning to enable them to attain and sustain a tenancy, enjoy a good quality of life, and avoid hospitalization or relapse.
  • Assessment and Planning: Assess referrals, formulate and review support plans, risk management plans, and move-on plans with clients to ensure their needs are met and their goals are achieved.
  • Collaboration and Partnership: Work in partnership with external agencies involved in the care and support of clients to ensure a comprehensive and coordinated approach to their needs.
  • Community Inclusion and Skills Development: Ensure clients are able to attend and participate in a range of socially inclusive activities and skills development opportunities to promote their independence and well-being.
  • Health and Wellness: Advise on and broker access to health services, education, training, and employment opportunities to support clients in achieving their health and wellness goals.
  • Teamwork and Flexibility: Work flexibly as part of the team to ensure service coverage and that contractual obligations are met.
Requirements
  • Experience and Skills: Possess experience of working with individuals with mental health and complex needs, and have a strong understanding of the principles of person-centered support and key-working.
  • Knowledge and Expertise: Have a good knowledge of welfare benefits, housing options, and legislation related to statutory duty, as well as health and safety relevant to working in clients' homes.
  • Communication and Interpersonal Skills: Possess excellent communication and interpersonal skills, with the ability to build strong relationships with clients, colleagues, and external agencies.
  • Continuous Learning and Development: Be committed to ongoing learning and development, with a willingness to undertake relevant training and professional development opportunities.
