About the Role

We are seeking a highly skilled and experienced Community Mental Health Practitioner to join our team in Mid Essex. As a Band 6 Community Nurse, you will be responsible for providing high-quality, evidence-based care to patients with dementia and other mental health conditions.

Key Responsibilities
  • Conduct comprehensive assessments and risk assessments to identify individual service user needs
  • Develop and implement personalized care plans in collaboration with the multidisciplinary team
  • Provide crisis interventions and home treatment to prevent hospital admissions
  • Work collaboratively with key partner organizations, service users, carers, and clinicians to deliver a 7-day a week service
  • Supervise and manage junior team members within the professional group and team locality
Requirements
  • Registered Mental Health Nurse or Registered General Nurse with a good standard of written and spoken English
  • Good standard of education and excellent IT skills
  • Current UK driving license and access to a car for work purposes
  • Experience in the performance management and supervision of lower-banded staff
