About the Role

We are seeking a highly skilled and dedicated Community Staff Nurse to join our team at Wirral Community Health and Care NHS Foundation Trust.

Main Responsibilities
  • Work as part of a multidisciplinary team to provide high-quality patient care in the community setting.
  • Develop and implement individualized care plans to meet the unique needs of patients.
  • Collaborate with other healthcare professionals to ensure seamless care transitions and optimal patient outcomes.
  • Conduct thorough assessments and evaluations to identify patient needs and develop effective care plans.
  • Provide education and support to patients and their families to promote health and well-being.
Requirements
  • Registered General Nurse educated to degree level (or equivalent)
  • Current NMC registration
  • Evidence of further recent professional development
  • Understanding of National Government initiatives in health and social care and how they impact on local services
  • Knowledge of a broad range of conditions experienced by patients in community and general practice settings
  • Excellent communication and interpersonal skills
  • Ability to work independently and as part of a team
  • Access to a car for work purposes
Desirable Qualifications and Experience
  • Community Nursing experience
  • Mentorship module or working towards
  • Experience working with patients with complex needs and conditions
