Job Title: Practice Nurse

Job Summary:

We are seeking a motivated and enthusiastic Practice Nurse to join our friendly and patient-focused team at Malmesbury Medical Partnership. As a key member of our multidisciplinary team, you will play a vital role in delivering high-quality patient care and contributing to the success of our practice.

Main Responsibilities:

  • Chronic disease management, including diagnosis, monitoring, and development of individual patient management plans using evidence-based practice.
  • Working as a senior member of the practice team, ensuring long-term condition management is delivered effectively to the entitled patient population.
  • Providing routine nursing care to patients as required, in accordance with clinical-based evidence, NICE, and the NSF.
  • Requesting and processing pathology results as required.
  • Maintaining accurate clinical records in conjunction with existing legislation.
  • Ensuring read codes are used effectively and supporting the achievement of QOF targets.
  • Supporting patients in the use of their prescribed medicines or over-the-counter medicines, within your scope of practice.
  • Liaising with external services/agencies to ensure patients are supported appropriately.

Secondary Responsibilities:

  • Providing wound care, including ulcer and Doppler care, to patients.
  • Providing immunisations, including childhood and seasonal vaccinations, to patients.
  • Providing cervical cytology screening.
  • Chaperoning patients where necessary.
  • Supporting the team in dealing with clinical emergencies and safeguarding matters.

Person Specification:

Essential:

  • Experience of working in primary care.

Desirable:

  • Experience of managing long-term conditions, including asthma, COPD, and diabetes.
  • Vaccination and immunisation training.
  • Wound management, including Doppler training.
  • Experience of TPP SystmOne.

Qualifications:

Essential:

  • Registered Nurse/HCP.
  • Chronic disease management training or commitment to training.

Desirable:

  • Non-medical prescriber.
