We are a large, friendly, CQC outstanding surgery, committed to quality improvement, based in modern purpose-built premises in Malmesbury.

We are a training practice, committed to development with a clinical team of 15 GPs and a nursing team of 9, supported by our PCN Specialist Practitioners, Clinical Pharmacists and Living Well Team as well as our highly skilled admin teams.

Job Summary

We are seeking a motivated and enthusiastic Practice Nurse/HCP with an interest in chronic disease management to join our friendly, patient-focused team.

This role will primarily be responsible for chronic disease management, including the diagnosis, monitoring, and development of patients' individual management plans using evidence-based practice.

Main Responsibilities

  • Ensure nursing services are delivered effectively to the entitled patient population.
  • Implement and evaluate individual treatment plans for chronic disease patients.
  • Identify, manage, and support patients at risk of developing long-term conditions, preventing adverse effects on patients' health.
  • Provide routine nursing care to patients as required in accordance with clinical-based evidence, NICE, and the NSF.
  • Request and process pathology results as required.
  • Maintain accurate clinical records in conjunction with existing legislation.
  • Ensure read codes are used effectively and support with achievement of QOF targets.
  • Support patients in the use of their prescribed medicines or over-the-counter medicines (within own scope of practice).
  • Liaise with external services/agencies to ensure the patient is supported appropriately (vulnerable patients etc.).

Secondary Responsibilities

  • Provide wound care (ulcer/Doppler etc.) to patients.
  • Provide immunisations (childhood, seasonal etc) to patients.
  • Provide Cervical Cytology screening.
  • Chaperone patients where necessary.
  • Support the team in dealing with clinical emergencies and safeguarding matters.

Person Specification

Experience

Essential

  • Experience of working in primary care.

Desirable

  • Experience of managing long-term conditions - Asthma, COPD, Diabetes.
  • Vaccination and Immunisation trained.
  • Wound management including Dopplers.
  • Experience of TPP SystmOne.

Qualifications

Essential

  • Registered Nurse/HCP.
  • Chronic Disease Management (or commitment to training).

Desirable

  • Non-Medical Prescriber.
