We are seeking a highly motivated and skilled Mental Health Practitioner (Band 6) to join our community-based service, providing mental health services for adults across Nottinghamshire. The successful candidate will work as part of our Local Mental Health Team, assessing, planning, and implementing personalized recovery paths for patients and their families.

Key Responsibilities:

  1. Caseload Management: Manage a caseload of patients under the Care Programme Approach, ensuring comprehensive assessments, treatment plans, and regular reviews.
  2. Assessment and Planning: Conduct thorough need/risk assessments, devise and implement treatment plans, and establish communication networks with clients, carers, and healthcare professionals.
  3. Interpersonal Skills: Utilize effective interpersonal skills to diffuse potentially emotive situations, ensuring a safe and supportive environment for patients and staff.
  4. Communication and Collaboration: Establish and maintain robust communication networks with clients, carers, team members, and other healthcare professionals to ensure seamless care coordination.

Requirements:

  • A full UK driving licence and vehicle for business use are required for this post.
  • Paid enhancements for unsocial hours working during night, weekends, and bank holidays.
