The successful candidate will work as part of our Local Mental Health Team, assessing, planning, and implementing personalized recovery paths for patients and their families.
Key Responsibilities:
- Caseload Management: Manage a caseload of patients under the Care Programme Approach, ensuring comprehensive assessments, treatment plans, and regular reviews.
- Assessment and Planning: Conduct thorough need/risk assessments, devise and implement treatment plans, and establish communication networks with clients, carers, and healthcare professionals.
- Interpersonal Skills: Utilize effective interpersonal skills to diffuse potentially emotive situations, ensuring a safe and supportive environment for patients and staff.
- Communication and Collaboration: Establish and maintain robust communication networks with clients, carers, team members, and other healthcare professionals to ensure seamless care coordination.
Requirements:
- A full UK driving licence and vehicle for business use are required for this post.
- Paid enhancements for unsocial hours working during night, weekends, and bank holidays.
