Job Summary

We are seeking a highly motivated and compassionate Healthcare Assistant to join our Community Nursing Team in West Norwich. As a key member of our team, you will play a vital role in providing high-quality care to patients in their homes, residential homes, and supported living complexes.

Main Responsibilities
  • Work under the supervision of a qualified practitioner to provide care to patients, following a prescribed treatment/care plan to prevent hospital admission.
  • Plan and prioritize visits to delegated patients, ensuring timely and effective care delivery.
  • Maintain accurate patient records, including SystmOne inputting, to ensure seamless care coordination.
  • Make basic judgments on patient responses to treatment/care plans, reporting findings to qualified practitioners.
  • Liaise with healthcare professionals to ensure comprehensive patient care.
  • Communicate effectively with patients, carers, and colleagues, demonstrating empathy and tact.
  • Establish and maintain positive working relationships with colleagues, patients, and other healthcare professionals.
  • Demonstrate dexterity and coordination when performing treatments, such as wound care and medication administration.
  • Ensure equipment is used safely and maintained appropriately, following relevant training.
  • Demonstrate awareness and understanding of consent, gaining consent as necessary.
  • Share responsibility for indirect patient contact tasks, such as answering phones and processing referrals.
  • Participate in health promotion and education activities, as required.
  • Work independently on specified tasks, with regular clinical support and supervision.

Person Specification
  • NVQ 2 or equivalent knowledge and experience.
  • Good general education, GCSE Grade 4/C or above in English and Maths or equivalent.
  • Experience of working within a healthcare setting.
  • Able to communicate effectively using the English language in both verbal and written form.
  • Good interpersonal skills.
  • Highly motivated.
  • Able to provide high standards of care.
  • Tact and diplomacy.
  • Empathy and sensitivity.
  • Ability to use own initiative.
