We are seeking a highly skilled and experienced Mental Health Practitioner to join our Intensive Outreach Team as a Clinical Lead. The successful candidate will be responsible for managing a small caseload of service-users who require intensive support and care.
Key Responsibilities- Assess, plan, deliver, and evaluate care for service-users referred to the Intensive Outreach Team, including risk assessment and management.
- Manage a caseload of patients as the named key worker/care coordinator/lead clinician, working collaboratively with service-users and their carers.
- Develop and implement individualized care plans, taking into account the service-user's needs, goals, and preferences.
- Work within a psychosocial/family-centered framework, promoting empowerment and self-advocacy.
- Collaborate with other agencies and disciplines to ensure comprehensive care and support.
- Participate in mental health promotion/education and provide advice to clients and carers on maintaining good health and preventing relapse.
- Administer and monitor the effects of prescribed medication, liaising with medical staff as necessary.
- Provide formal clinical supervision to students and other staff within the team.
- Participate in the team's duty rota, taking duty calls as required and following the lone working policy.
- Current professional registration with an appropriate body in healthcare or social work.
- Extensive experience in care coordinating people in a community mental health setting.
- Experience working in the community under an Assertive Outreach Team (AOT) model.
