We are seeking a highly skilled and compassionate Care Coordinator to join our team at Rural Warwickshire North PCN. As a Care Coordinator, you will play a vital role in providing personalized support to patients and their families, ensuring that their social and psychological needs are met.
Key Responsibilities- Manage and prioritize a caseload of patients, providing proactive and responsive support to meet their needs.
- Work closely with member practices, pharmacies, multi-disciplinary teams, and allied health professionals to ensure seamless care coordination.
- Utilize population health intelligence to identify patients who require personalized care approaches, and develop and implement effective care plans.
- Facilitate and ensure the effective delivery of patient-centered, personalized health and social care plans, monitoring progress and reporting outcomes.
- Provide coordination and navigation for patients and their carers across the spectrum of services available, including hospital discharge and community care.
- Support local Care Homes to manage patient needs, including completing and reviewing Care Home Care Plans and Dementia reviews as required.
- Undertake assistance with clinician visits to see patients in their own homes, where applicable, for Blood Pressure checks, welfare checks, and provide support.
- Work alongside other PCN Additional Roles staff and outside agencies to provide wrap-around support to patients.
- Make appropriate third-party referrals, and complete and maintain accurate patient records.
- Ability to manage and prioritize a caseload, and work flexibly and enthusiastically within a team or on own initiative.
- Excellent communication skills, both written and verbal, with the ability to build relationships with patients, their families, and carers.
- Ability to provide personalized support to individuals, their families, and carers, and to listen and empathize with people in a non-judgmental way.
- Full UK driving license and access to a car, with the ability to travel independently between practice sites and attend meetings hosted by other agencies.
