About the Role

We are seeking a highly skilled and compassionate Community Outreach Worker to join our team at Hertfordshire Mind Network. As a key member of our team, you will play a vital role in supporting refugees and asylum seekers in the community with multiple needs who have mental health as a primary need.

Key Responsibilities
  • Provide advice, information, and holistic support to clients with mental ill health, ensuring their safety and wellbeing are monitored and reviewed regularly.
  • Support clients to build social networks and reduce isolation, promoting self-management through building an understanding of their own triggers and coping strategies.
  • Work closely with key delivery partners across the system to ensure the service is responsive, inclusive, and easy to access.
  • Take allocated referrals from various agencies, including the Community Navigators, Borough and District Councils, and self-referrals.
  • Hold a caseload of clients, supporting them for approximately 10 weeks, and provide safeguarding intervention support as needed.
  • Provide advocacy services to clients experiencing mental ill health and assist them in accessing appropriate mental health services, housing/tenancy services, and other services according to their assessed needs.
