Job Title: Community Heart Failure Specialist Nurse

We are seeking a highly skilled and experienced Community Heart Failure Specialist Nurse to join our team at Central and North West London NHS Foundation Trust. As a key member of our multidisciplinary team, you will play a vital role in providing high-quality care to patients with heart failure in their own homes or in a clinical setting.

Main Responsibilities:
  • Optimise guideline-directed medical therapies in line with current NICE guidance (Chronic Heart Failure)
  • Manage fluid balance and care of patients in a clinic setting or in their own homes
  • Work collaboratively with local hospital acute trusts, other community services, and primary care services across Camden
  • Contribute to the development of the team by ensuring policies and procedures are up-to-date and evidence-based
Requirements:
  • Registered Adult Nurse with NMC registration
  • Numeracy skills required for safe titration of heart failure medications
  • Evidence of relevant Continuous Professional Development
  • Education to degree level with evidence of an established degree pathway
  • Practice Assessor/Practice Supervisor training
  • History Taking and Physical Assessment Course
Desirable Skills and Knowledge:
  • Ability to communicate effectively with the multidisciplinary team, present cases, and update others in the team
  • Maintaining accurate, timely, and concise clinical records
  • Ability to work in partnership with patients, families, and other healthcare professionals
  • Understanding of the Mental Capacity Act and Deprivation of Liberty Safeguards
  • Understanding of the Mental Health Act
Working Arrangements:

This is a full-time Band 7 role with a highly functioning and well-supported Community Heart Failure Service. You will work in partnership with local hospital acute trusts, other community services, and primary care services across Camden.

The ideal candidate will have a broad cardiology knowledge base, excellent assessment skills, and treatment of people with heart failure. You will have the opportunity to work with a wide variety of community and secondary care teams, aimed at reducing unscheduled hospital admissions and improving quality of life for people with heart failure.
