Key Responsibilities- Provide person-centred support to Service Users, promoting their independence and well-being.
- Develop and implement individualised support plans, taking into account the unique needs and goals of each Service User.
- Work collaboratively with other professionals, including healthcare teams and social workers, to ensure comprehensive support.
- Contribute to a safe and supportive environment, adhering to risk assessments and management strategies.
- Participate in regular training and professional development to enhance skills and knowledge.
- Experience working with adults or children with learning disabilities.
- NVQ level 2 in Health and Social Care or equivalent.
- Ability to work flexibly, including weekends and bank holidays.
- Driver's license (desirable).
- Competitive hourly rate: £12.59-£12.80.
