Job Summary:

We are seeking an experienced Physiotherapist to join our dynamic inpatient rehabilitation teams at a community hospital. The successful candidate will provide high-quality care and rehabilitation to patients with a range of functional and rehabilitative needs.

About the Role:

The Physiotherapist will be responsible for providing expert clinical advice and therapy to patients with complex conditions living in the community. This will involve working closely with other healthcare professionals to develop and implement individualized care plans that promote independence and improve quality of life.

Key Responsibilities:

  • Provide clinical Physiotherapy skills within the Community Therapy Team/Community Hospital Team(s), and all other provider organisations.
  • Develop and implement individualized care plans that promote independence and improve quality of life.
  • Work closely with other healthcare professionals to ensure seamless care delivery.
  • Contribute to the development and implementation of rehabilitation programs that meet the needs of patients with complex conditions.

Requirements:

  • Registration with the Health and Care Professions Council (HCPC)
  • Rehabilitation experience
  • Previous experience of working in a community setting
