About the Role
We are seeking an experienced Occupational Therapist to join our Learning Disability Team at Reading Borough Council. As an Occupational Therapist with our team, you will play a vital role in supporting individuals with learning disabilities and neurodiversity across various age groups.
Main Responsibilities
- Provide timely transitions for Care Act eligible 14- to 18-year-olds into adulthood, ensuring all adults with a learning disability and/or autism receive appropriate ASC support under care act eligibility criteria.
- Promote independence and achievable outcomes for young and older adults.
- Embed an enablement focus, supporting individuals to remain in their own homes and maximize their independence.
- Develop close relationships with families, ensuring they understand the role and remit of ASC for eligible LDA adults.
- Collaborate with providers, community and voluntary services, children's services, schools, and colleges to achieve optimal outcomes.
- Use a strengths-based approach and OT skills to support individuals in finding solutions, promoting wellbeing and healthy occupations through various interventions.
Requirements
- A degree in Occupational Therapy.
- Registered with HCPC.
