Job Title: Advanced Care Practitioner

We are seeking an experienced Advanced Care Practitioner to join our team in East Lancashire. The successful candidate will be responsible for coordinating palliative care services and supporting patients and their families in end-of-life care.

About the Role

The Advanced Care Practitioner will work closely with GPs, nurses, and other healthcare professionals to identify individuals who may be at the end of their life and provide support and guidance to ensure they receive the best possible care.

The role will involve:

  • Liaising with GPs to promote the use of the EARLY clinical search tool to identify patients who may be in the last 12 months of their life
  • Supporting the clinical validation process and reviewing patients who may benefit from advance care planning discussions
  • Acting as a core member of the PCN MDT and liaising with other providers of palliative and end-of-life care to refer and/or signpost to other services
  • Coordinating palliative care register meetings and ensuring individuals have access to equitable care
  • Formulating individual advance care plans and ensuring patients and their families are central to the future care shared decision-making process
Requirements

To be successful in this role, you will need:

  • Registered General Nurse qualification
  • Advanced Communication Skills (ACST) training or equivalent
  • 1st degree in a related field
  • Recent evidence of related study
  • MSc Palliative and End of Life Care qualification
