Job Summary

This is an exciting opportunity to work in a dynamic and supportive environment, providing holistic care and support to children and young people with complex health care needs. As a Care Team Member, you will work closely with our families to develop and implement individualized care plans, and provide emotional and social support to help them navigate the challenges of caring for a child with a life-limiting condition.

Main Responsibilities

As a Care Team Member, your key responsibilities will include:

  • Providing high-quality, person-centered care to children and young people with complex health care needs
  • Developing and implementing individualized care plans in collaboration with our families and healthcare professionals
  • Providing emotional and social support to our families, including counseling and advocacy
  • Working collaboratively with our multidisciplinary team to ensure seamless care and support
  • Participating in ongoing professional development and training to maintain and enhance your skills and knowledge

Requirements

To be successful in this role, you will need:

  • NVQ 2/3 in Care or a related field
  • Safeguarding Awareness Level 1
  • Experience of working with children and young people with complex health care needs
  • Excellent communication and interpersonal skills
  • A flexible and adaptable approach to work
