Carmel Medical Practice is seeking a skilled Medication Management Specialist to become an integral part of our healthcare team. The primary responsibilities of this role encompass a blend of clinical, technical, and administrative functions under the guidance of the Practice Pharmacist. The successful candidate will play a crucial role in ensuring the optimal utilization of medications, which includes conducting thorough medication reviews, reconciling discharge documentation, and providing patient support for medication-related inquiries.

This position offers flexibility in terms of full-time or part-time arrangements, which can be discussed during the interview process. Opportunities for hybrid working may also be available.

Key Responsibilities

- Managing prescription requests efficiently

- Coding and processing diagnoses from clinical and discharge documents

- Assisting with medication adjustments based on clinical recommendations

- Collaborating with the pharmacist to conduct High-Risk Drug Monitoring Audits and scheduling necessary appointments

- Staying informed on Medicines Safety Audits and processing relevant results

- Addressing prescription-related queries from patients

- Coordinating medication synchronization efforts

- Monitoring controlled drug prescriptions and managing orders

- Facilitating medication requests from healthcare professionals as appropriate

- Supporting Quality and Outcomes Framework initiatives

- Conducting medication review monitoring to ensure compliance with clinical guidelines

- Updating and managing ophthalmology-related medications

- Leading Medicines Optimisation projects aligned with local healthcare initiatives

- Verifying formulary updates and implementing cost-effective medication alternatives

- Providing alternatives for medications that are out of stock

- Assisting in transitioning medications to liquid forms when necessary

Clinical Duties

- Engaging in patient-facing roles to promote effective medication use through collaborative discussions

- Performing medication optimization tasks, including administration checks and supporting medication reviews

- Utilizing consultation skills to partner with patients for effective medication management

- Supporting medication reviews and reconciliations for new patients and during transitions of care

- Offering specialized expertise to address public health and social care needs, including lifestyle guidance

- Playing a key role in shared care protocols and liaising with specialist pharmacists for complex cases

- Promoting antimicrobial stewardship to minimize inappropriate antibiotic use

- Assisting in the execution of medicines optimization and patient safety audits

- Supporting the implementation of national prescribing policies within primary care settings

Technical and Administrative Duties

- Collaborating with the Practice Pharmacist and pharmacy teams to enhance medication management systems

- Supervising reception teams in processing prescription requests and implementing clinical monitoring

- Leading initiatives for optimizing medication systems

- Providing training on the safe handling of medications and the Electronic Prescription Service

- Fostering professional relationships with pharmacy and healthcare professionals in the community
