Collaborating closely with service users, families, and other multi-agency partners, you will play a vital role in supporting recovery and social inclusion for our clients. A strong emphasis is placed on preventative work to empower individuals.
Key Responsibilities:
- Provide information, guidance, and support to individuals, fostering their independence and helping them achieve their goals.
- Work collaboratively with service users, families, and multi-agency partners to develop and implement effective care plans.
- Maintain comprehensive and accurate case records through clear and concise written communication.
- Contribute to a supportive and collaborative team environment.
