We are seeking an experienced Social Worker to join our innovative and supportive Hospital in-reach Adult Social Care Pathway. The successful candidate will have a strong background in working with adults with complex mental health needs and their carers.
Main Responsibilities- Advocate and work as part of a multi-disciplinary team to provide active involvement in the assessment, development, implementation, and evaluation of service user care.
- Utilize evidence-based interventions to develop support and services appropriate to individuals' needs, utilizing strength-based approaches.
- Work collaboratively and effectively with adults with complex mental health needs, providing a sound working knowledge of the Mental Health Act, Care Act, Mental Capacity Act, and other legal and statutory frameworks.
- Provide direct support to the Practice Lead and supervise and peer support to the team.
