Job Title: Specialist Community Health Visitor

Job Summary:

We are seeking a highly skilled and experienced Specialist Community Health Visitor to join our team at Central London Community Healthcare NHS Trust. As a Specialist Community Health Visitor, you will play a vital role in delivering high-quality health visiting services to clients in a variety of community settings.

Main Responsibilities:

  • Provide skilled, effective, and evidence-based health visiting services to clients in community settings.
  • Apply specialist skills and knowledge in the delivery of the health visiting service.
  • Consult, advise, and educate individuals and families on good health care practices.
  • Develop health profiles of the defined population and local communities, identifying health needs of disadvantaged groups and targeting outreach services to meet their needs.
  • Identify, prioritize, and implement programs of care that meet the public health needs of the population.
  • Provide parenting skills support and information through coordinating/participating in parenting groups and one-to-one home visits to families, especially those with complex health needs/vulnerabilities.
  • Provide health assessments, advice, and education to families and individuals of all ages in the population group, prioritizing services appropriate to local public health and population need.
  • Develop and implement regular health promotion initiatives on specific health issues.

Requirements:

  • Current NMC registration.
  • Evidence of continuing post-registration education and training.
  • Recent health visiting experience.
  • Ability to coordinate health promotion initiatives in line with local and national health improvement targets.
  • Experience of assessing, planning, and implementing programs of care and writing support to others.
  • Experience in providing practice-based learning support to others.
