Mental Health Support Worker

We are seeking a highly motivated and experienced Support Worker to join our Mental Health and Complex Needs service in Kingston Upon Thames. As a Support Worker, you will play a crucial role in supporting service users with mental health and dual diagnoses to access the community, develop independent living skills, and achieve their goals.
Key Responsibilities- Support service users with mental health and dual diagnoses to access the community, develop independent living skills, and achieve their goals.
- Assess referrals, formulate and review support plans, risk management plans, and move-on plans with service users.
- Work in partnership with outside agencies involved in the care and support of service users.
- Ensure service users are able to attend and participate in a range of socially inclusive activities and skills development opportunities.
- Advise on and broker access to health services, education, training, and employment opportunities.
- Work flexibly as part of the team to ensure service coverage and that contractual obligations are met.
- Experience of working with people with mental health and dual diagnoses.
- Experience of delivering high-quality support services to people with complex needs.
- Previous experience of using computer-based systems to accurately record work and client details.
- Good knowledge of welfare benefits and housing options and legislation related to statutory duty.
- Knowledge of health and safety relevant to working in service users' homes.
- Ability to undertake client-focused key working, support planning, and risk assessments to maximise service users' independence.
- Good understanding of the range of issues, diagnoses, and treatment relevant to the client group, including substance misuse.
- Ability to liaise with external professionals to inform and develop service users' support packages.
- Willingness to actively engage in undertaking relevant personal development and training.
- Ability to travel throughout the Borough of operation.
