About the Role

We are seeking a highly skilled and compassionate Care Team Leader to join our team at Acuity Care Group Limited. As a Care Team Leader, you will be responsible for providing exceptional care and support to individuals with complex needs, including those with learning disabilities, autism, and mental health conditions.

Key Responsibilities

  • Provide high-quality care and support to individuals with complex needs, including those with learning disabilities, autism, and mental health conditions.
  • Lead and manage a team of support workers to ensure the delivery of excellent care and support.
  • Develop and implement individual care plans that meet the unique needs and goals of each individual.
  • Ensure the safety and well-being of individuals at all times, including during sleep-ins and other activities.
  • Communicate effectively with individuals, families, and other stakeholders to ensure seamless care and support.
  • Collaborate with other teams and services to ensure comprehensive care and support.
  • Participate in ongoing training and development to enhance skills and knowledge.

Requirements

  • Team leader experience within the healthcare sector.
  • Autism experience.
  • Learning disability experience.
  • Mental health experience.
  • Ability to effectively communicate with individuals requiring care.
  • Strong attention to detail and adherence to safety protocols.
  • Empathy, patience, and compassion when interacting with individuals in need of support.
  • Flexibility and adaptability to meet varying care needs.
  • Ability to work collaboratively as part of a team.
