Job Summary

We are seeking an experienced Advanced Clinical Practitioner to join our team at Bridge Medical Centre in Crawley, West Sussex. As a key member of our primary care team, you will play a vital role in delivering high-quality patient care and contributing to the success of our practice.

Main Responsibilities
  • Assess, diagnose, and treat patients presenting with minor illnesses and acute conditions.
  • Undertake home visits for acute conditions, as assessed by your duty doctor, and within the boundaries of your professional qualifications.
  • Use advanced clinical skills to provide education to patients, promoting self-care and empowering them to make informed choices about their treatment.
  • Collaborate with our multidisciplinary team, including GPs, Paramedic Practitioners, Advanced Nurse Practitioners, Practice Nurses, and HCAs, to deliver comprehensive patient care.
  • Participate in the audit process, evaluation, and implementation of plans and practice change to meet patient needs.
Requirements
  • Qualified Advanced Nurse/Clinical Practitioner with evidence of an appropriate knowledge base.
  • Experience of working in an urgent care/emergency setting and primary care setting.
  • Ability to work within a team to provide the best outcome for patients.
  • Competent use of IT systems, including Excel, Word, and Outlook.
  • Flexible attitude to work, with a willingness to work later/earlier as duties/events require.
