We are seeking a dedicated and compassionate individual to support people in their own homes, helping them to achieve their full potential and live independently in their community.

As an Assistant Reablement Coordinator, you will work closely with our reablement team to deliver a high-quality service, promoting and sustaining people's independence in the community.

Key Responsibilities:

  • Support service users in their own homes to achieve their goals and rebuild their independence
  • Manage daily living skills and set clear, achievable goals to help people sustain independent living
  • Work collaboratively with the reablement team to deliver a high-quality service
  • Ensure that service users' journeys are well-coordinated and compliant with organisational policies and Care Quality Commission regulated guidance
