Assistant Reablement Coordinator

We are seeking a dedicated and compassionate Assistant Reablement Coordinator to join our team. As a key member of our reablement team, you will play a vital role in supporting individuals to live independently in their own homes.

About the Role

As an Assistant Reablement Coordinator, you will work closely with service users, their carers, and families to identify and address their needs. You will develop and implement personalized support plans, focusing on promoting independence, confidence, and well-being.

Key Responsibilities

  • Deliver high-quality reablement services, ensuring a seamless and coordinated approach to supporting service users.
  • Work collaboratively with the reablement team to achieve the best possible outcomes for service users, their carers, and families.
  • Conduct thorough assessments, identifying areas for support and developing tailored plans to address these needs.
  • Provide emotional support, guidance, and education to service users, empowering them to make informed decisions about their care.
  • Maintain accurate records, adhering to organizational policies, procedures, and Care Quality Commission regulated guidance.
