Occupational Therapist - Discharge to Assess

Hunter AHP is seeking an experienced Occupational Therapist to join their Discharge to Assess team in Bedford. As an Occupational Therapist, you will play a crucial role in assessing the needs of patients and facilitating their discharge back home within 28 days.

Key Responsibilities:

  • Assess patient needs and develop personalized plans for discharge
  • Provide equipment and access visits to support patient independence
  • Collaborate with the Discharge to Assess team to plan and facilitate patient discharge
  • Work closely with the wider healthcare team to ensure seamless patient care

Requirements:

  • Experience as an Occupational Therapist in a Discharge to Assess role
  • Strong assessment and planning skills
  • Ability to work independently and as part of a team
  • Excellent communication and interpersonal skills
