About the Role

We are seeking an experienced and self-motivated nurse to join our Acute Care at Home Team based at Truro Health Park. As a Specialist Practice Community Acute Care Senior Staff Nurse, you will be part of a multi-disciplinary team delivering high-quality, evidence-based care to acutely ill patients in their homes, with the aim of preventing hospital admission or facilitating rapid discharge.

Main Responsibilities
  • Liaise with referring hospitals or GPs to arrange admission to the scheme
  • Assess, develop, implement, and evaluate patient care
  • Work on a flexible rota shift system
  • Deputise for the Team Leader and assist in the management of the team

Person Specification

Essential Qualifications

  • RGN 1st level registration or equivalent
  • Diploma level specialist training, experience, or short courses (or equivalent experience)

Desirable Qualifications

  • Prescribing qualification
  • Teaching qualification, eg, City & Guilds, HEA Mentorship module (or working towards)
  • 1st Degree Diploma or working towards

Essential Experience

  • Working without direct supervision, yet as a team member through experience and education
  • Substantial post-registration experience in Acute or Community Teams
  • Experience of a wide range of medical conditions
  • Evidence of autonomous practice
