We are seeking an experienced and self-motivated nurse to join our Acute Care at Home Team based at Truro Health Park. As a Specialist Practice Community Acute Care Senior Staff Nurse, you will be part of a multi-disciplinary team delivering high-quality, evidence-based care to acutely ill patients in their homes, with the aim of preventing hospital admission or facilitating rapid discharge.
Main Responsibilities- Liaise with referring hospitals or GPs to arrange admission to the scheme
- Assess, develop, implement, and evaluate patient care
- Work on a flexible rota shift system
- Deputise for the Team Leader and assist in the management of the team

Person SpecificationEssential Qualifications
- RGN 1st level registration or equivalent
- Diploma level specialist training, experience, or short courses (or equivalent experience)
Desirable Qualifications
- Prescribing qualification
- Teaching qualification, eg, City & Guilds, HEA Mentorship module (or working towards)
- 1st Degree Diploma or working towards

- Working without direct supervision, yet as a team member through experience and education
- Substantial post-registration experience in Acute or Community Teams
